Health History

EPractice Medical® enables you to easily view and add health history information for a specific individual.

Problems: outlined list of problem name, additional descriptive text, discovery date, onset date, hospitalization dates, resolution status, resolution date, and security level to view problem.  You can view all diagnosis, procedures, tests/orders, medications, ticklers, and progress notes for each problem.

Assessments : color code outline of all risk factors (green, yellow, red) for a specific person based on answers to the subjective and objective questions in the contacts.  Assessments can be viewed for a specific date range and grouped by category, question, or answer.

Alerts: a list of quality measures that the specific individual meets. The alerts are setup in the administration program.

Contacts: all contacts for a specific person including whether the contact has been printed, reviewed, signed, or completed; contact type, date, provider, and primary diagnosis.
Measurements: all measurements for a specific person including date, height, weight, BMI, BMI percentile, BMI category, head circumference, and two user measurement variables. You also have the ability to print height, weight, BMI, and head circumference growth charts.

Vital Signs: all vital signs for a specific person with date, blood pressure, temperature, respiratory rate, and pulse rate.

Medications: all medications prescribed or dispensed to a specific person including medication name, date, dose number, per day, number of days prescribed, flag for chronic medications, comments. Prescriptions can be printed.
Immunizations: all immunizations for a specific person including immunization, date, provider, site.

Family History: family history information for a specific person including condition, relationship, onset age, and comment.

PERSON OR STAFF FOLLOW-UP TICKLERS


EPractice Medical ® enables you to easily record and follow-up on tickler information for a specific individual or staff member. Reminders, referrals, and lab test results can be displayed for specific time intervals and status (pending, overdue, completed). The appointments are directly linked to the new scheduling module.


  • Reminders:
    a grid containing all reminders including type of reminder, person or provider, due date, time, priority status (importance of the reminder), notes, completion date, follow-up notes. New reminders can be added.

  • Referrals : a grid containing all referrals including type of referral, person or referred by, referred to, due date, time, priority status, notes, completion date, follow-up notes. New referrals can be added.

  • Appointments: a grid containing all appointments including type of appointment, person or provider, due date, time, priority status, notes, completion date, follow-up notes. New appointments can be added. The appointments can be connected to the scheduling module.  .

  • Lab Tests: a grid containing all lab tests for a specific person including lab test, date, result, comments.
